NEA National Heritage Fellowships Nomination Form
To make a nomination, simply enter the information requested below, then click
the Submit button.
A nominee must be a U.S. citizen or permanent resident of the United States.
Because this is a Web form, it does not have all of the features of a word
processing program (i.e., you cannot format your text with bold or italics, and
you cannot use the spelling check feature). Use quotation marks or capital
letters to set off titles of works or productions. We suggest that you create
all text using your preferred word processing program and then copy it into the

practicing, and contributions to the folk and traditional
arts. For the Bess Lomax Hawes Award – briefly describe
how this individual has fostered excellence, vitality,
and public appreciation of traditional arts and artists.

citing major public appearances, exhibitions, published
works, etc. You may also provide references and links to
recent articles written about the nominee(s), if any.
